Hormonal Therapy for GI Angiodysplasia: Out of Hope After Failure of the Scope?

Ujjwal Kumar, Kevin Cowley and Frederick Weber

Article Type: Case Report | First Published: September 14, 2015

Angiodysplasia of the gastrointestinal (GI) tract can present both diagnostic and therapeutic challenges. Despite the recent advances in endoscopy, many patients continue to suffer from recurrent bleeding. We report a remarkable case of a multi-year history of transfusion-requiring obscure overt recurrent GI bleeding, refractory to multiple endoscopic therapies as well as both octreotide and thalidomide. Upon initiation of hormonal therapy, the patient's bleeding completely resolved, with comple...

Hoda M. Malatyr, Bincy Abraham, Mina S. Emil, George D. Ferry and Elizabeth A. Garnett

Article Type: Research Article | First Published: September 11, 2015

The incidence of inflammatory bowel disease (IBD) in children has increased over the past 20 years and treatment with biologics is increasingly used. Objective: We assessed the associations of biologics to IBD outcomes in children. Methods: A retrospective investigation was conducted on a cohort of children with IBD between 1988/2008 who were enrolled in the Pediatric IBD registry. The diagnosis of IBD was based on clinical, radiological, endoscopic, and/or histological examinations. Clini...

Savvas Psarelis, Sophia Corsava, Kyriakos Polycarpou, Kyriakos Ioannou, Ioanna Zouvani and Elena Nikiphorou

Article Type: Case Report | First Published: September 05, 2015

We present the case of a 38-year-old male patientwho presented with new onset nephropathy on a background of psoriatic skin and joint disease. The pathogenesis and link between nephropathy with psoriatic disease is discussed, along with potential diagnostic and management challenges. This case highlights that nephropathy in psoriatic skin and joint disease can and does occur and high vigilance is necessary especially in patients taking potentially nephrotoxic drugs. The complexity of these cases...

A Case of Psoriasis in an HIV Positive Male

Omar F Merchant and Wayne X Shandera

Article Type: Case Report | First Published: August 29, 2015

Patients with Human Immunodeficiency Virus (HIV) infection have an increased risk for several variants of psoriasis as well as being refractory to standard treatment regimens. While psoriasis does not affect HIV survival, quality of life may be significantly impaired and these considerations warrant special attention with management. We report a case of psoriasis in a 43-year-old Hispanic male with HIV infection, adequately managed with antiretroviral therapy. He presented with chronic and persi...

Mycotic Aneurysm of Native Renal Artery: Case Report and Review of the Literature

Arru Luca, De Magistris Luigi, Arendt Charlie, Pillet Jeanchristophe, Azagra Juan Santiago, Goergen Martine and Pouthier Dominique

Article Type: Case Report | First Published: August 28, 2015

Introduction: Renal native artery is an uncommon localisation of mycotic aneurysm that involves most frequently the artery of renal transplant in immuno-suppressed patients. Septic embolization often occurs during endocarditis and direct infection could complicate endovascular procedures with device placing. Case report: We herein describe the case of a leaking mycotic aneurysm of the native renal artery (MANRA) in a renal transplanted patient, successfully treated by open aneurysmectomy and ...

Sural and Radial Sensory Responses in Patients with Sensory Polyneuropathy

Ying Guo, J Lynn Palmer, Xun S Brown and Jack Fu

Article Type: Review Article | First Published: August 21, 2015

The amplitude of Sensory Nerve Action Potentials (SNAPs), as an indicator for the amount of peripheral sensory nerve axon, is important in the diagnosis of peripheral neuropathy. Since the sural and radial nerves are at low risk for compressive injury, the sural and radial SNAPs are especially useful in the electrodiagnosis of polyneuropathy....

Hepatic Arterioportal Fistulae: Two Case Reports and Review of the Literature

Thanh Tran, Marc Burrell and William M. Lee

Article Type: Case Report | First Published: August 20, 2015

Hepatic arterio portal fistulae are inappropriate connections between the hepatic artery and portal vein and are obscure causes of acquired portal hypertension. There have been a small number of cases reported since it was first described in 1892 by Sachs. These fistulae can cause significant morbidity and mortality as a result of the portal hypertension, but unlike cirrhosis, are potentially curable. Therefore, early diagnosis and treatment is important. The two cases presented here illustrate ...

Primary malignant fibrous histiocytoma (MFH) localized in the large intestine is extremely rare. We report a first case of MFH in colon that underwent a laparoscopy-assisted colectomy. A 74-year-old female had a 3-month history of blood stool. Barium enema study showed protrusion in the descending colon. Colonoscopy revealed submucosal tumor-like lesion with central ulceration. A biopsy was obtained and the pathological report was that MFH was suspected. Laparoscopic-assisted left hemicolectomy ...
