Is Post-Operative High Dependency Admission Required for Children with Obstructive Sleep Apnea after Adenotonsillectomy?

Chu Qin PHUA, Yi Rong Leonora LIU, Pei Yuan FONG, Kay Yee Winnie FUNG and Kun Kiaang Henry TAN

Article Type: Retrospective study | First Published: April 29, 2019

The commonest cause of paediatric obstructive sleep apnoea (OSA) is the disproportionate adenotonsillar hypertrophy in relation to craniofacial growth and airway calibre. In recent years, there is an increasing awareness of paediatric OSA, leading to increasing number of adenotonsillectomy being performed for this indication. Adenotonsillectomy has been shown to confer improvement in child behaviour and quality of life. In up to 79% of patients, it can lead to normalisation of polysomnography....

Physical Approach towards the Problem of Aging

Berezin AA

Article Type: Commentary | First Published: April 29, 2019

In the beginning of the century L. Boltzmann was the first to represent the increasing entropy process as a time flowing characteristic. Besides, he pointed at a possible time reversion process in micro systems. A symbol of aging of any system - alive or inanimate is the entropy increasing. Is this process reversible? For short periods - yes and biological systems represent examples of it. Unfortunately, any reversing of the entropy sign in these systems is possible only for short periods of time and then the aging continues. There are many physical processes of this kind. This is a laser in a...

Inter-Arm Blood Pressure Difference in a Typical University Family Medicine Clinic

Kent H Do, MD, DPT, Capt, USAF, MSC, Sorana Raiciulescu MSc and Jeffrey C Leggit MD, CAQSM, Col (Ret), USA, MC

Article Type: Research Article | First Published: April 29, 2019

The purpose of this study was to determine the point prevalence of Inter-Arm Blood Pressure Difference (IAD) in a University Family Health Center (UFHC). 217 patients seen by the Uniformed Services University's (USU) UFHC were assessed in this non-randomized screening study. Blood pressure was measured simultaneously in both arms with automated sphygmomanometers in a seated position during normal scheduled appointment screening process after 5 minutes of sitting. The measure was repeated after one minute. The main outcome measure was to establish the point prevalence of either a systolic or di...

Inflammatory Bowel Diseases (IBD) - Associated Spondyloarthropathy in the Biologic Era: A Case Series

Cristina Tocia, MD, Luana Alexandrescu, MD, Victoria Cristina Șuța, MD and Eugen Dumitru, MD

Article Type: Case Series | First Published: April 29, 2019

Articular manifestations are the most common extraintestinal manifestations in inflammatory bowel diseases and are classified into axial spondyloarthritis (SpA) and peripheral SpA. EIM may occur before or after IBD diagnosis, being the proof that IBD is not only limited to gut. Here we present a case series of three patients with Crohn's disease and extraintestinal manifestations admitted to our hospital. Patients had in common young age, Crohn's disease with moderate or severe clinic and endoscopic activity, axial spondyloarthropathy and some particularities: One case associated type 1 periph...

Liver Transplant with a Donor after Bothropic Accident - Case Report

Reslem Feitosa Custódio, Matheus De Souza Mendes, Maximilian Pinho Schwermann, Ivelise, Ivelise Regina Canite Brasil, Jose Eudes Bastos Pinho and Andrea Benevides Leite

Article Type: Case Report | First Published: April 29, 2019

Orthotopic Liver transplantation is the primary treatment for most end-stage-liver diseases. Victims of snakebite are potential organ donors since the venom's ability to induce irreversible brain damage. However, in the literature, reports of liver transplants obtained from victims of these accidents, their outcome and follow up are practically non-existent. Our purpose is to report a case of a donor with brain death following complications of a bothropic accident, and the patient who receives the liver with cirrhosis due to Hepatitis B....

Surgical Treatment of a Young Athlete with Displaced Avulsion Fracture of the Anterior Superior Iliac Spine (ASIS)

Alexander E Poor, Alec Warren, Nicole Cannon and William C Meyers

Article Type: Case Report | First Published: April 29, 2019

Avulsion fractures of the anterior superior iliac spine are uncommon without established treatment guidelines. Deciding between nonoperative versus surgical management depends on the grade of displacement, skeletal maturity, type of sport/activity, and competitive/career requirements. We present a young athlete who sustained an ASIS avulsion fracture while playing baseball that was treated with open surgical reduction and fixation using absorbable sutures with an emphasis on mobilizing the TFL to reapproximate the avulsed segment without tension....

Proteinuria Level and Associated Changes in Glomerular Podocytes and Renal Tubular Epithelium

Maha M Arafah

Article Type: Original Research Article | First Published: April 29, 2019

The correlation between glomerular podocytes and renal tubular cells involved in handling of leaked proteins and the level of proteinuria was evaluated. Retrospective 22 cases of clinical proteinuria in patients with various glomerulopathies were retrieved and analyzed. Glomerulopathies in the concerned patients were pathologically diagnosed through light and electron microscopic examination of the submitted renal biopsies. Three cases with protein levels in urine within the acceptable normal range were additionally analyzed as controls. Electron microscopic examination of the glomerular podoc...

A Comparative Study of Three Classification Procedures: Asthma among Healthcare Professionals in Texas

Dejian Lai, Ahmed A Arif, Haiyun Xu and George L Delclos

Article Type: Original Research article | First Published: April 29, 2019

Statistical classification analysis has been widely used in many fields. In this article, we applied and compared three different classification procedures: Logistic regression, Fisher''s linear discriminant function and the second order Bahadur representation to two datasets from two surveys on asthma among healthcare professional in Texas. The first dataset contained 102 subjects and the second dataset 2963. The concordance of the classification from the three statistical procedures with possible asthma identified by physician and airway responsiveness to methacholine challenge was assessed ...

The Impact of the Rigid Catheter Guide on Trocar Injury during Mid-Urethral Sling Placement

Amin Tavakoli, BS, Azadeh Nasiri, MD and Felicia Lane, MD

Article Type: Research article | First Published: April 29, 2019

Our goal was to identify general risk factors associated with trocar bladder injury during a retropubic mid-urethral sling (MUS) placement and evaluate the effect of using the rigid catheter guide on cystotomy rates. This is a retrospective cohort with nested case control study of 291 patients who underwent a MUS procedure between January 2002 and December 2012 at a single academic medical center. Logistic regression was used to conduct multivariate analysis to predict bladder injury....

What is New with Bordetella?

Shamsuddin Anwar MD, Allison Glaser MD and Mahreen Anwar

Article Type: Case Report | First Published: April 29, 2019

A 27-year-old HIV-1 positive male was admitted in September 2018 for sudden onset of vomiting and diarrhea and was conservatively managed for positive Norovirus on a Gastrointestinal PCR panel. It was anticipated to start him on ibalizumab intravenous infusions and enfuvirtide upon discharge and a PICC line was placed in the right internal jugular vein. On the third day of line placement, he developed a fever with mild respiratory symptoms. His respiratory PCR panel for Rhino/Enterovirus was positive and blood cultures grew Enterobacter. Blood cultures from PICC line confirmed the diagnosis of...
