Effect of Vitamin D Level and Physical Exercise on the Physical Performance and Functional Test Results in Elderly Women

Neide AP Nascimento, Patricia FP Moreira, Vanessa A Carvalho, Levy Aragao, Rosangela Villa Marin-Mio, Marise Lazaretti-Castro and Maysa S Cendoroglo

Article Type: RESEARCH ARTICLE | First Published: January 23, 2019

Global awareness on vitamin D deficiency and its consequences has been increasing. Furthermore, there is substantial agreement among all guidelines that serum 25(OH)D concentrations < 25-30 nmol/L should be avoided in all age groups. In addition, hypovitaminosis D is a world health problem, and Brazil has an elevated prevalence of hypovitaminosis D among its population. In general, a 25(OH)D level of less than 20 ng/mL has been associated with neuromuscular performance issues in elderly people including poor physical performance, low muscular strength, and muscle weakness, thus suggesting that...

Could we Spare Carbapenems for Empiric Therapy in ESBL-Enterobacteriaceae Colonized Elderly with Community-Onset Sepsis?

Article Type: RESEARCH ARTICLE | First Published: January 21, 2019

The preference for empiric antibiotic therapy with carbapenems over non-carbapenems in all extended-spectrum β-lactamase-producing Enterobacteriaceae (ESBL-E) colonized patients with sepsis warrants further investigation from an ecologic perspective; even more so in the elderly (≥ 65 years) because the proportion of ESBL-E in adults increases with patients' age. In this retrospective observational study, enrolling 547 ESBL-E colonized elderly patients with community-onset sepsis, hospitalized at a single medical centre from 2011 to 2015, the positive predictive value of ESBL-E faecal coloni...

Stigma Related to Fear and Shame Restricts Access to HIV Testing and Treatment in Tanzania

Danielle A Walker, Kyle L Johnson and Jacen S Moore

Article Type: Review Article | First Published: February 22, 2019

Tanzania is one of 15 nations that share 75% of the HIV burden in sub-Saharan Africa, with a national prevalence of 5.3% and prevalence rates as high as 30% in special populations. Knowledge about HIV infection, testing, and treatment is low, especially in younger populations, suggesting a need for effective HIV educational programs. We sought to understand the impact of HIV-related stigma and gender roles in HIV education and knowledge in educational programs and explored methodologies that effectively incorporate culturally competent approaches to reduce stigma in HIV education programs....

Article Type: Case Report | First Published: February 22, 2019

Congenital adrenal hyperplasia (CAH) is a rare autosomal recessive disorder with mutations in genes involved in cortisol and aldosterone production. Based on overall 21-OHase activity, CAH is divided into classic (C-CAH) and non-classic (NC-CAH). Females who suffered from NC-CAH have had increased infertility rates and higher miscarriage susceptibility. The treatment of CAH in pregnancy is still debatable. We present 22 years old pregnant female (seventh week of gestation), who is currently under dexamethasone (DEX) since almost seven years for NC-CAH. At presentation, she is normotensive, non...

Early Removal of Foley Catheter after Sigmoid Colectomy for Diverticular Colovesical Fistula without Intraoperative Bladder Repair or Postoperative Cystography: Feasibility of a Quality Improvement Pilot Program

Anthony Carden, MD, Riley K Kitamura, MD, John Leppert, MD and Dan Eisenberg, MD

Article Type: Review Article | First Published: February 20, 2019

The treatment of colovesical fistula (CVF) due to diverticular disease is complex and imposes significant risk to the patient. Specifically, management of the bladder defect after fistula takedown is inconstant. In this quality improvement study, we report on the safety of early (< 7 days) urethral catheter removal without intraoperative or postoperative bladder imaging. Between 2008 and 2018, patients who were operated on for CVF due to diverticular disease were identified retrospectively. Medical records were reviewed to obtain patient characteristics, operative technique and findings, and p...

Determining Rheumatology Patient Interest in a Group Strength Training Program - Results of an Exercise Survey

Kevin Rhie, MD, Danielle M Feger, MS, Rayford R June, MD, Christopher N Sciamanna, MD, MPH and Sharon E Banks, DO, MS

Article Type: Research Article | First Published: February 20, 2019

Exercise has proven benefits in rheumatologic disease including reducing inflammation and improving symptoms. A Group Strength Training (GST) program design has improved adherence to exercise in primary care patients but the effect is unknown in rheumatology patients. We examined the interest of rheumatology patients with different diagnoses and the effect of comorbidities in pursuing an organized GST program. We conducted a cross-sectional survey of patients from a rheumatology practice in central Pennsylvania in February and April 2017. This survey assessed self-reported interest of patients...

Dental Caries in Geriatric Dentistry: An Update for Clinicians

Rapp L, Maret D, Diemer F and Lacoste Ferré MH

Article Type: Review Article | First Published: February 20, 2019

Caries is a progressive infectious disease that, without treatment, leads to the destruction of the dental organ and can damage the surrounding tissues. According to the WHO, it is the third global scourge affecting all ages of life. Over the past 40 years, prevention campaigns for children and adolescents have led to a decline in the prevalence of caries. The CAOD index for children aged 6 to 12 years decreased from 4.20 in 1987 to 1.23 in 2006 and there is an increase in the number of children who have no cavities. The impact of these preventive actions is to preserve teeth throughout life. ...

Spontaneous Coronary Artery Dissection: An up to Date

Dario Buccheri and Daniele Adorno

Article Type: Commentary article | First Published: February 20, 2019

Spontaneous dissection of the coronary artery (SCAD) is an uncommon cause of acute coronary syndrome (ACS) and is defined as a dissection of the coronary artery not associated with an atherosclerotic process, plaque rupture and/or thrombus formation and, by definition, not iatrogenic. The resulting myocardial damage is determined not by the rupture of a plaque indeed, but by the formation of an intramural hematoma (IMH) or intimal/medial disruption that determines the dissection of the arterial wall with the formation of a false lumen that obstructs the blood flow through the true lumen. SCAD ...
