Moreno Diaz Javier, GarcĂa Brunen Jose Miguel, Loscos Aranda Silvia and Arenas Miquelez Aranzazu Inmaculada

Article Type: CASE REPOR | First Published: September 07, 2018

Lemierre's syndrome is a rare complication of tonsillitis, with septic emboli and jugular vein thrombosis. We present a case in a young adult without immune deficiencies that required purulent collection drainage and admission to the Intensive Care Unit. This entity, although rare, is potentially fatal and should be known for early diagnosis and treatment....

An Alternative Mesofemoral Shunt for Uncontrolled Variceal Bleeding

Yi-Ju Wu, Chih-Che Lin, Yu-Hung Lin, Chao-Long Chen, Wayne Huang and Yu-Feng Kao

Article Type: ORIGINAL ARTICLE | First Published: September 07, 2018

Portal hypertension can lead to life-threatening variceal bleeding (VB). Transjugular intrahepatic portosystemic shunt (TIPSS) is the standard choice for uncontrolled VB. Unavailable for TIPSS, we design a mesofemoral shunt (MFS) as an alternative method for control of VB. From March 2011 to November 2014, eleven patients with VB due to liver cirrhosis were enrolled. The MFS was created by connecting from superior mesenteric vein below transverse colon, via subcutaneous tunnel, to right femoral vein with an 8-mm diameter polytetrafluoroethylene (PTFE) graft....

Giant Brunner's Gland Adenoma - An Unusual Cause of Gastrointestinal Haemorrhage

F Mahmood, Adil Ahmed, Chung Shen, Sinead Cabezon, and Vittal Rao

Article Type: CASE REPORT | First Published: September 07, 2018

Brunner's gland adenoma is a rare benign tumour of the duodenum. Less than 200 cases have been reported in the literature. Usually asymptomatic, these lesions rarely manifest as upper gastrointestinal haemorrhage or duodenal obstruction. We report a 40-year-old gentleman who presented with melaena and iron deficiency anaemia. Duodenoscopy revealed a large pedunculated tumour in the duodenal bulb. ...

Houda Ben Ayed, Makram Koubaa, Chakib Marrakchi, Khaoula Rekik, Fatma Hammami, Fatma Smaoui, Mariem Ben Hmida, Sourour Yaich, Imed Maaloul, Jamel Damak and Mounir Ben Jemaa

Article Type: REVIEW ARTICLE | First Published: September 03, 2018

Tuberculosis (TB) is one of the major public health threats, competing with the human immunodeficiency virus (HIV) as the cause of death due to infectious diseases worldwide. Extrapulmonary tuberculosis (EPTB) contributes to the burden of disease and does not receive specific attention in international control strategies. We aimed to give an update on the current epidemiological data of EPTB, to describe its clinical spectrum, to identify the main risk factors and to propose effective preventive strategies to reduce EPTB burden. Recent epidemiological data showed that the proportion of EPTB ar...

Monitoring the Time Course of Disability through a Self-Assessment Instrument "Activity Index" (IA) in RA Patients

L Ren, X Zhang, ZG Li, H Tang and R Theiler

Article Type: ORIGINAL RESEARCH | First Published: September 01, 2018

Rheumatoid arthritis (RA) is a heterogeneous autoimmune disease whose etiopathogenesis is largely unknown. Available treatments, though effective, are insufficient in so far as there is no cure for a major proportion of patients. In those patients the disease becomes chronic with progressive joint damage, disability, and limitation of participation. Current treatment approaches include pain-relieving drugs and anti-inflammatory medications that slow joint damage, combined with physical therapies involving a well-balanced, highly personalized sequence of rest and exercise. As pain-relief, overa...

Article Type: CASE REPORT | First Published: August 30, 2018

Tarsal navicular bone dislocation is a rare condition which can lead to long term disability, mainly due to difficulties to obtain an anatomical reduction. Only few cases have been reported. We report a case of a 21-year-old Cameroonian male who was admitted to the emergency department after a road traffic accident. The physical examination reveals a patient unable to bear weight, with a painful s...

Implantation of Pacemaker in a Patient with Dextrocardia, Persistent Left Superior Vena Cava, TGA and Heart Block

Article Type: CASE REPORT | First Published: August 29, 2018

Congenital anomalies of the heart may generate difficulties for cardiac invasive procedures. Dextrocardia is a rare congenital heart disease in which the apex of the heart is located on the right side of the chest. Patients with transposition of great arteries (TGA) abnormalities are at risk for complete cardiac block and usually require a permanent cardiac pacemaker in long-term follow-up. The pe...

The Ketogenic Diet and its Clinical Applications in Type I and II Diabetes

Raffaele Pilla

Article Type: Review Article | First Published: September 06, 2018

It has been shown that metabolic-based therapies, such as nutritional ketosis, are effective to contrast seizure disorders and various acute and chronic neurological disorders. From a physiological perspective, glucose is the primary metabolic fuel for cells. However, many neurodegenerative disorders have been recently associated with impaired glucose transport and metabolism and with mitochondrial dysfunction causing energy deficits, such as in Alzheimer's disease, Parkinson's disease, general seizure disorders, and traumatic brain injury....

Type 2 diabetes mellitus (T2DM) previously known as non-insulin dependent diabetes mellitus is the most common type of diabetes mellitus. The rapid increasing prevalence, high morbidity and mortality of the disease must be encountered by an increase in scientific research. Animal models are very important in the preclinical studies to validate the use of new drugs. Streptozotocin - nicotinamide (S...
