BMI ≥ 40 kg/m2 without comorbid disease

BMI 35-39.9 kg/m2 with ≥ 1 serious comorbidity, e.g.:

- Type 2 Diabetes Mellitus

- Hypertension

- Hypercholesterolemia

- Obstructive Sleep Apnea

- Gastroesophageal Reflux Disease

- Nonalcoholic Steatohepatitis

BMI 30-34.9 kg/m2 with ≥ 1 serious comorbidity, e.g.:

- Uncontrollable type 2 Diabetes Mellitus

- Metabolic Syndrome

Bariatric surgery is controversial in patients < 18 or > 65

- Considered when comorbidities are severe
